Forms to Fill Out & Send In
- HEALTH FORM—This form should have been sent in with the application. We cannot take any student from the airport if we don’t receive this prior to their arrival. We must have the original and cannot accept faxed or emailed copies.
- DOMESTIC TRAVEL PLANS FORM—We need this information before your student flies to Chicago in order to know where and when to pick them up. Please send it in right away once you’ve secured your domestic ticket in April/May. Important Reminder: Do not book your domestic ticket until you receive the Updated Mission Trip Schedule that we send out in mid-April.
- PHYSICIAN FORM—We will need this to be partially filled out by your physician.
- TERMS, AGREEMENTS, AND LIABILITY FORM—This form must be signed and if under 18 signed by both parents and by a Notary Public. We must have the original form and cannot accept faxed or emailed copies.
- TRAVEL CONSULTANT & UNACCOMPANIED MINOR FORM—For students 15 or under. Please read this form carefully and send it in prior to their arrival in Chicago if they are required by the airlines to travel as an unaccompanied minor.
- MENTOR PROGRAM FORM
- TRAVEL INSURANCE OPTION—Overseas travel insurance is required. An option was enclosed in the Prep Packet. If selected, this form gets mailed to Bursch Travel not Reign Ministries.
- PRESCRIPTION INFO SHEETS—This is included with your students prep packet and should be filled out for each prescription and sent with student to Training Camp.

- APPLY FOR YOUR PASSPORT: May 1st
- CHURCH SUPPORT DUE: May 15th (suggested)
- HEALTH FORM, TERMS & AGREEMENT, PHYSICIAN FORM, UNACCOMPANIED MINOR (IF APPLICABLE) AND DOMESTIC TRAVEL PLANS: May 23rd
- PASSPORT PHOTOCOPY: June 1st (black and white copy, NOT COLOR, of the page with students’ name and picture. It is illegal to make color copies of passports.)
- STAFF DISCIPLER ARRIVAL: June 15/16, 2012
- STUDENT ARRIVAL: June 22/23, 2012
- OVERSEAS MISSIONS TRIP FLIGHT DATE: July 1/2 – depending on trip
- REMAINING SUPPORT DUE: June 27th
