Forms

There are six forms that need to be filled out and mailed back to Royal Servants:

• Health Form
• Physician Form
• Terms, Agreements, & Liability Form
• Domestic Travel Plans Form
• Unaccompanied Minor Form
• Mentor Form

Each is important.  We encourage you to read through each form.  The Health Form and Skills Groups Form were included in your application.  All other forms are included in the Prep Packet.  They can also be downloaded using the links below.

Note: The Health Form was to be sent in with the application.  Most other forms are time-sensitive and must be returned to Royal Servants by a specified date.  Please refer to the Reference Sheet included in the Parent Packet.  Additionally, there are forms that require a parent’s signature that need to be signed if you are under 18.

In regard to a passport, be sure to fill out the form included in your Prep Packet and bring it in to your local passport agency.  We recommend this is done by May 1, if possible.  Your parents will need to accompany you to the passport agency if you are under 18.  It will take 4-6 weeks for you to receive the passport in the mail.  If you are traveling to a country that requires a visa (China, Nepal, Sierra Leone, India) it is important that the travel date on the passport application is the end of May.  This will allow us the time necessary to obtain the visas for each participant.

Note: If you are under 18 years old at the start of the summer, your parents are required to sign the Terms, Agreements, and Liabilities form and have it notarized.  The form is located below, and was included in your Pre Packet.  This notarization is official proof that it is your signature on the form, and that your parent of guardian has given permission to be on a Royal Servants trip.  We will need this proof should a passport be lost or stolen, and we have to replace it overseas.  Additionally, a number of foreign countries are starting to require notarized release forms before they allow a person to enter the country.
